Overview

This annual 2.5-day CME-accredited conference is designed to enhance clinical practice by fostering interdisciplinary collaboration. By bringing together physicians, advanced practice nurses, physician assistants, pharmacists, and healthcare administrators, the event promotes seamless communication between primary and specialty care. The goal is to reduce care fragmentation, improve continuity, and ultimately elevate patient outcomes across the region. Through shared learning and strategic dialogue, participants gain practical insights to drive more efficient and coordinated patient management.   

The focus is to provide learners with proven treatment options for disease states prevalent in our region. Specialists will address challenges in managing these maladies and discuss the variation of treatments required in properly maintaining the patient’s health specifically in the patient with multiple diagnoses. The learner will be provided content that is supported by evidence-based therapies with some examples of proven outcomes data within the NMHS system.

The agenda is under development; final faculty, topics, and times are subject to change. 


ACCME Objectives
  1. Describe communication strategies to improve coordination between primary and specialty care providers.
  2. Identify opportunities to reduce fragmentation in patient care across diverse healthcare settings.
  3. Apply evidence-based practices to improve clinical decision-making and patient outcomes.
  4. Evaluate system-level approaches to address regional healthcare challenges and disparities.
  5. Strengthen leadership and teamwork skills among healthcare professionals to support integrated care delivery.
  6. Recognize the clinical indications, risks and potential for misuse associated with commonly prescribed controlled substances.
  7. Apply best practices for prescribing, monitoring, and documenting controlled substances to ensure patient safety and regulatory compliance.

ANCC Learning Outcomes
  1. Learners will self-report through the post evaluation ways in which they plan to implement into practice the prescribing laws and regulations for the safe and effective use of controlled substances in the management of the complex patient.
  2. Learners will self-report through the post evaluation ways in which they plan to implement into practice the multidisciplinary team roles into the management of patients who require specialty care to improve care coordination and patient outcomes.

Accreditation

 

 The Mississippi State Medical Association is a member of the Southern States CME Collaborative (SSCC).  North Mississippi Medical Center is accredited by the SSCC to provide continuing medical education for physicians. 

NMMC designates this Live Activity course for a maximum of 23.00 AMA PRA Category 1 Credit(s)TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

NMMC is approved as a provider of nursing continuing professional development by the Mississippi Nurses Foundation, Inc., an accredited approver by the American Nurses Credentialing Centers Commission on Accreditation.  This activity has been awarded for 23.00 contact hour(s). CE#00192-EDI-2430.

Successful completion of this CME Activity, which includes participation in the evaluation component, enables the participant to earn up to 18.5 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
