Comprehensive care for patients with advanced heart disease is robust and requires heavily layered treatment plans, which present challenges for primary care physicians, advance practice providers and specialists. This activity is designed to educate the health care team regarding the impact of advanced heart disease on patients, clarify the recommended role of supportive care, and explore strategies for supporting patients and caregivers through a continuum of care.

1. Review the impact of advanced heart disease, particularly chronic heart failure, on patients
2. Clarify the recommended role of supportive care in advanced heart failure
3. Explore strategies for supporting patients with advanced heart failure and their caregivers through the continuum of care within our health system

1. All learners will report through the post-evaluation an increase in knowledge of the impact of advanced heart disease and heart failure.
2. All learners will report through the post-evaluation an intent to incorporate into practice strategies for supporting patients with advanced heart failure and their caregivers through the continuum of care within our health system.

NMMC is approved as a provider of nursing continuing professional development by the Mississippi Nurses Foundation, an accredited approver by the American Nurses Credentialing Centers Commission on Accreditation.  This activity has been awarded for 1.00 contact hour(s). CE#00192-EDII-2377.

Cancellation Policy
